Request new developments or support on the TPN system

A little bit of a stupid question please? In terms of requesting a bank code, could you please advise how to fill in the screen in terms of days, amounts, period, etc. What must I enter if I'm taking up a lease with a tenant at R5000pm, for 12 months.
